Process of appointment to staff following a lapse in appointment

I have a physician who allowed his appointment/privileges to expire. Now a month later wishes to be reappointed. How is this handled? We would like to reappoint the physician but when I was educated for my position I was informed that if a physician's priveleges and appointment expire/lapse, then the physician is required to fill in the full application and all education/training, experience must be reverified. Is this current practice? Or can one do the shorter "reappointment" process that does not require reverifiication of education/training/all past affilitations, etc.

When this happens we have the physician update his prior application, sign and date it. Than process by verifiying the current affiliations/insurances/peer references/license - no need to verify education though. Than the application will go thru the committee process as a "new applicant"

We require the practitioner to complete the full application again (and pay the application fee again), however things that are static, like medical school, internship, and residency do not have to be reverified.
